Citizenship Application (N-400)
1. A photcopy of both sides of your Permanent Resident Card
2. Two identical color photographs with your name and Alien Registration Number (A-Number) in pencil on back of each photo
3. Check or money order for the application fee ($595) and the biometrics services fee for fingerprinting ($85) $680 Total. Write your A-Number on the back of the check or money order.

Petition for Alien Fiancée (I-129F)
1. Proof that you are a US citizen: birth certificate if born in the United States, front and back copy of your original Certificate of Naturalization if you were naturalized, or Original Certificate of Citizenship.
2. Proof that you can legally marry: Evidence that you and your fiancée have meet within the last 2 years or if you have not meet provide detailed explanation and evidence to why not. Provide original statements from you and your fiancée and copies of any evidence you wish to submit to establish your mutual intent. If either you or your Fiancée were married before give copies of documents showing that each prior marriage was legally terminated. 
3. Submit a completed and signed Form G-325A (Biographic Information) for you and your fiancée.
4. Give USCIS a passport-style color photograph of yourself and your fiancée, with both photos taken within 30 days of the date of filing this petition. The photos must have a white background, be glossy, un-retouched and not mounted. The dimension of the full frontal facial image of you and your fiancée in separate photos should be about one inch from your chin to the top of your hair. Using a pencil or felt pen, lightly print the name (and Alien Registration Number, if known) on the back of each photograph.
5. Filing Fee $340

Petition for Alien Relative (I-130)
1. Proof that you are a US citizen: birth certificate if born in the United States, front and back copy of your original Certificate of Naturalization if you were naturalized, or Original Certificate of Citizenship.
2. Proof of family relationship
3. Filing Fee is $420

Investor Visa (I-194A)
Provide a detailed statement which describes for the last fiscal year:
1. The aggregate amount of EB-5 alien capital invested through your regional center [Form I-924A Supplement, Part 3, No.1];
2. The aggregate number of new direct and/or indirect jobs created by EB-5 investors through your regional center [Form I-924A Supplement, Part 3, No.1];
3. the aggregate number of jobs "maintained" jobs by EB-5 capital investments into a "troubled business" through your regional center, if applicable [Form I-924A Supplement, Part 3, No.1];
4. The industry(s) that have been the focus of EB-5 capital investments sponsored through the regional center, and the resulting aggregate EB-5 capital investment and job creation. (Note: Separately identify jobs maintained through investments in "troubled businesses") [Form I-924 Supplement, Part 3, No.2];
5. The names, addresses, and industry category title of each job creating commercial enterprise located within the geographic scope of your regional center that has received alien investor capital. Also, provide the aggregate amount of EB-5 capital investment, the aggregate number of new direct and/or indirect jobs created by EB-5 investors,and if applicable, the aggregate number of jobs that have been "maintained" through EB-5 capital investments into a "troubled business", for each commercial enterprise located within the geographic scope of your regional center [Form I-924A Supplement, Part 3, No.3];
6. If the EB-5 Commercial Enterprise(s) serve as a vehicle for investment into other business entities that have or will create or maintain jobs for EB-5 purposes, then please identify the name, addresses, amount of EB-5 capital investment, and the number of jobs created or maintained by the actual job creating businesses through EB-5 investments [Form I-924A Supplement, Part 3, No.3];
7. The total number of approved, denied, or revoked Form I-526 petitions filed by EB-5 alien investors, for capital investments sponsored through your regional center [Form I-924A Supplement, Part 3, No.4];
8. The total number of approved, denied, or revoked Form I-829 petitions filed by EB-5 alien investors, for capital investments sponsored through your regional center [Form I-924A Supplement, Part 3, No.5]

Investor Visa (I-194)
Evidence requirements for filing:
1. A request for the Approval and Designation of a regional center
2. The regional center must focus on a geographical area. This area must be contiguous can clearly identified in the application by providing a detailed map of the proposed geographic area of the Regional Center.
3. Each Regional Center must fully explain how at least 10 new full-time jobs will be created by each individual alien investor within the Regional Center either directly or indirectly. 
4. Provide a detailed description of the past, current, and future promotional activities for the regional center. Include a description of the budget for this activity, along with evidence of the funds committed to the regional center for promotional activities. 
Submit a plan of operation for the regional center which addresses how investors will be recruited and how the regional center will conduct its due diligence to ensure that all immigrant investor funds affiliated with its capital investment projects will be obtained from lawful sources. 
5. Provide a general prediction which addresses the prospective impact of the capital investment projects sponsored by the regional center, regionally or nationally, with respect to increases in household earnings; greater demand for business service, utilities, maintenance and repair; and construction both within and without the regional center. 
6. The application must fully describe and document the organizational structure of the regional center. In addition, it is helpful for the regional center to show that the capital investment offering instruments, business structure, and operating agreements of the proposed commercial enterprises that will be affiliated with the regional center are compliant with the EB-5 statutory and regulatory requirements, as well as the binding EB-5 precedent decisions.
